Work Process

A physician or healthcare facility refers the patient. We conduct an initial assessment to understand the patient's needs and develop a personalized healthcare plan. Authorization and Plan Approval

The agency verifies insurance coverage and obtains necessary authorizations. The physician reviews and approves the care plan.

A care team is assigned, and services begin with the first home visit. Ongoing visits and adjustments to the at-home healthcare plan are made as needed, keeping the physician and family informed.

Periodic assessments are conducted to evaluate progress and adjust the Healthcare plan. When goals are met, a transition plan is developed, and the patient is discharged, with follow-up support if necessary.
