Position: Physical Therapist Contractor

Responsibilities:

• Conduct comprehensive evaluations of referred patients to develop personalized treatment plans.
• Provide effective and compassionate physical therapy treatments to help patients regain mobility and independence.
• Supervise patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Coordinate with other healthcare professionals to ensure cohesive and comprehensive care.
• Document patient progress and maintain accurate records using EMR systems.

Compensation:

• Competitive pay ranging from $60.00 to $80.00 per visit, commensurate with experience and qualifications.

Experience:

• Preferred: 1-2 years of experience in a home health setting.
• New graduates with a passion for home health are encouraged to apply!

Requirements:

• Valid Physical Therapy license in the State of Illinois.
• Current CPR certification (classroom-based).
• Valid driver’s license.
• Recent physical exam and TB test.
• Familiarity with Therapy Boss and Kinnser EMR systems is an advantage but not required.
